Partners in Your Child’s Health

For Children and Adolescents (From birth to 20 years 11 months old)

Georgia NurseCare has more than 25 years of experience in providing proven pediatric care for children in southeast Georgia. We offer skilled and non-skilled pediatric services through a Georgia Medicaid Waiver Program known as GAPP. GAPP stands for Georgia Pediatric Program. GAPP services are designed to serve eligible members who have complex medical conditions. Members of the GAPP program must have medically necessary nursing and/or personal care support needs for services to be approved.

A child may qualify for in-home nursing services if the child:

  • Is ventilator dependent

  • Has a tracheotomy

  • Receives continuous nutritional feeds through a G tube or NG tube

  • Is oxygen dependent

  • Has other complex medical needs such as seizures, cerebral palsy, SMA, cardiac disease, pulmonary hypertension, or brain injuries

Personal Care Support is also provided to eligible members who need assistance with:

  • Feeding

  • Bathing

  • Dressing

  • Personal hygiene

  • Meal preparation

  • Light housekeeping

  • Mobility and ambulation

  • Or other covered services ordered in the member’s plan of care.

The member’s plan of care for nursing services and personal care support must be developed by the member’s primary physician and approved by the state.
